Enrolment Forms for New Zealand Residents

1. Download and Complete - Application for Enrolment at Liston College (New Zealand Residents)
2. Copy of Passport or Birth Certificate (original to be sighted at time of interview)
3. Download and Complete Preference of Enrolment Certificate Form (if applicable)

5. Interview with the Principal will be arranged for student and parent / caregiver by Lyanne Evans on acceptance by the Principal – the following documentation will be sent out to the parent / caregiver:

• Letter of Acceptance
• Automatic Payment Form (to be filled out if applicable)
• Liston College Computer and Internet Acceptable Use Agreement – Students
• Telecom School Connection Sponsor Sign-Up Form (optional)
• $250 to be paid as part payment of the 1st Term (or the Term that the student starts at Liston College) either by cash , eftpos, cheque, or automatic payment
